GM Healthier You Patient Transition Letter

PLEASE READ THIS IMPORTANT INFORMATION ABOUT HEALTHIER YOU, NHS DIABETES PREVENTION PROGRAMME.

As you’re at risk of developing diabetes your GP referred you to Healthier You the Diabetes Prevention Programme. Reed Wellbeing will be providing Healthier You from August. So that you can join Reed’s programme we need to provide them with the information your GP gave us.

If you don’t want us to share the information provided by your GP with Reed Wellbeing please let us know before 23^rd September and you will be discharged from the service. Let us know by:

If we don’t hear from you, we will assume you’re happy for us to send Reed Wellbeing your referral information. This will be done in a secure way as required by the NHS on the 26^th September. 

To comply with NHS England guidelines or information will be held with us for a limited time.
