Direct Referral

Direct referral is easy using the referral form embedded on your clinical system.

Practice staff can refer patients directly by sending the completed referral form to XYLA Health & Wellbeing (formerly ICS Health & Wellbeing) at scwcsu.greatermanchester@nhs.net. 

You can refer following:

• Contact with your patients
• At an annual review
• An NHS Health Check

System alerts are being installed on clinical systems and will remind you when a patient is eligible to join the programme.

The referral form was installed at your practice by the GMSS Data Quality Team named ‘NDPP Healthier You Referral Form’. This will be saved where documents are usually saved in your clinical system. If you cannot locate the form, please email gmss.dataqualitygm@nhs.net including your practice name and clinical system in use so that the correct version can be sent to you to import.
