XYLA Health & Wellbeing (formerly ICS Health & Wellbeing) Group Based Service
XYLA Health & Wellbeing (formerly ICS Health & Wellbeing) deliver the group based service in Greater Manchester. The service uses a research-based curriculum and trained health and wellbeing coaches. You can trust that your patients will receive evidence-based information about healthy nutrition and exercise support and other important lifestyle changes.
Programme Curriculum
The nine-month programme includes an initial one-to-one assessment followed by 13 group sessions and a final one-to-one review at the end of the programme.
Initial Assessment
An initial one to one assessment will be held with a Health and Wellbeing Coach to assess and understand their individual needs, and motivations. Patients will be given an overview of the programme and produce a personalised action plan to set their long and short-term goals.
6 fortnightly sessions:
Session 1 Understanding diabetes
Session 2 What is a balanced lifestyle?
Session 3 Thinking about getting active?
Session 4 What is a healthy diet?
Session 5 Practical solutions for a healthier you
Session 6 Enjoying my physical activity
Followed by 7 monthly sessions:
Session 7 Know your health
Session 8 Values and obstacles
Session 9 Stress, mindfulness and mindful eating
Session 10 Physical activity within my community
Session 11 Habits, self-compassion and visualisation
Session 12 Sleep and creating your health blueprint
Session 13 Designing my physical activity plan
End of Programme Review
Patients will receive an end of programme one-to-one review with their Health and Wellbeing Coach to celebrate and evaluate their progress and set post-programme goals. Participants will be signposted to other appropriate services available to them, advising them to speak with their GP for onward pathway referrals.
